This retrospective study aimed to determine the prevalence and factors associated with respiratory tract infections among pediatric patients who presented at Nnamdi Azikiwe University Teaching Hospital, Neni over 5 years (January 2017-December 2021). Of 874 under-five visits, respiratory tract infections were recorded in 36.8% (n322). Slightly higher cases occurred in the dry 176 (54.66%) versus wet season 146 (45.34%), with a spike during the December-March harmattan. Males were more affected (64.29%) than females (35.71%). Infants 1 year had the highest prevalence (45.34%) while 4-year-olds had the lowest (8.07%), indicating risk decreases with age. Respiratory tract infections persist as a major public health problem among Nigerian children despite efforts to reduce under-five infectious disease morbidity and mortality. The prevalence varies by gender, age, and season - peak harmattan periods and highest in infants - reflecting differential risk factors. Tailored interventions for the highest-risk groups are needed to curb respiratory morbidity and associated mortality. More research into geospatial, socioeconomic, nutritional, and environmental factors could illuminate other drivers of pediatric respiratory infections amendable to preventive strategies. Ultimately, context-specific understanding is key to mitigating this persistent threat to under-five well-being in Nigeria and similar settings.
