This study aimed to determine the prevalence and factors influencing pica practices amongst 326 pregnant women attending antenatal clinics at Nnamdi Azikiwe University Teaching Hospital, Anambra State, Nigeria. A cross-sectional descriptive study was conducted from September to November 2022 using systematic sampling and interviewer-administered questionnaires after obtaining ethical approval. Data analysis was done with SPSS version 25.0. The mean age of respondents was 29.3 years. Overall pica prevalence was 25.8% in the current pregnancy. The most identified determinants were smell (53.6%) and taste (47.6%) of favored pica items. Geophagy was the dominant form (72.6%), with white clay as the most frequently consumed substance. Educational level did not significantly impact pica practice. Pica practices play a substantial role in maternal and fetal nutrition and health. The specific pica behaviors are influenced by regional variations in the availability of substances. Healthcare professionals and NGOs must intensify health education on the deleterious maternal-fetal effects, including inadequate nutrition. Further studies should explore the reasons for pica's persistence despite education. Urgent action is needed to mitigate and prevent pica and its consequences amongst this vulnerable population.
